ADVANTAGE Choice Plans are a popular alternative to our HMO and POS plans and include the ADVANTAGE HMO affiliated networks PLUS a preferred provider organization (PPO)... giving employers the benefit of offering their employees more choice through a broader state-wide network of providers.

Plan Features:

  • Employee selects either the PPO network or an HMO network at time of enrollment
  • Health plan benefits are the same regardless of network chosen
  • Employee pays a higher percent of the premium if the PPO network is selected
  • Members can go to most in-network specialists without a PCP referral
  • PCPs are available to help navigate the health care system
  • Disease Management and complex case management are included without additional cost
  • Members can enroll in the Mountain of Wellness program
  • Employers can actively engage in benefit plan designing
  • Employee and dependents must select the same network

Underwriting Guidelines:

  • Available to groups with 51 or more employees
  • 80% of employees must reside in the ADVANTAGE HMO service area
  • Premium rates are approximately 15% higher for employees selecting the PPO network and funded through employee contributions
